Criminal Record Check - signed by parents
Entry Forms - signed by parents
Temporary appointment of guardianship - signed by parents in presence of notary public or comissionaire of oaths.

Athletes need these ID at border crossing:
SGI photo, Birth certificate, treaty or metis cards, health cards. Athletes need these pieces of ID to keep on themselves at all times. While on route and at all times in Denver. I will also need copies of these ID as soon as possible. Please fax them to me.
